Care Convene is a simple and easy way to accomplish your medical appointments from the privacy of your home. The app based product will allow you to have a face to face appointment with a member of your medical team from your iPad, iPhone, or Android device.

We will be using these electronic visits to make it easier for follow-up appointments. The type of appointment we well schedule will include: medication change/start follow-up, pain management, diabetes, hypertension, depression/anxiety, behavioral health, and other appointment at the discretion of the providers.

All you have to do is follow these few easy steps:

• Go to your App store and download “Care Convene”
• Go to Register Now to create your account. This includes: First and Last Name, Dat of Birth, State, Mobile Number, Email Address, Password, and a Provider Code.
• Once you have created your account, you will need to verify your email address by following the instructions in an email sent to you by Care Convene app.
• Use your email and password to log in to the Care Convene app.
  • Choose who the appointment is for.
  • Chose the care procider
  • Reason for the visit
  • Onset Date will be today’s date
  • Additional details: you will enter the reason for your visit (medication follow-up, diabetes follow-up, etc.)
  • Then click on “Request eVisit”
    • This will let me know you are waiting.

Once the provider has started the appointment, the patient and provider will be able to text back and forth and video chat is needed.

Once the eVisit is completed, a $45 charge will be sent to your insurance. You will be responsible for covering the remaining balance after insurance.

Please disregard appointment reminders from FollowMyHealth as you do not need to show up to the office for these appointments.
